vs 57% of the quinidine patients (p < 0.0001). Sixty-eight percent of the flecainide patients met the above criterion and also had complete suppression of couplets and beats of ventricular tachycardia, vs 33% of the quinidine patients (p < 0.0001). PR and QRS intervals were prolonged by flecainide without clinical consequence, but they were not substantially affected by quinidine (p < 0.0001). Quinidine prolonged JT (QT minus QRS) intervals significantly more than flecainide (p < 0.05). Nineteen of 141 flecainide patients and 21 of 139 quinidine patients discontinued therapy because of side effects (p > 0.50). Flecainide side efects included dizziness, blurred vision, headache and nausea. Quinidine side effects included diarrhea,

Quinidine-related mortality in the short-to-medium-term treatment of ventricular arrhythmias. A meta-analysis.

BACKGROUND The interim results of the Cardiac Arrhythmia Suppression Trial requires physicians to use a higher threshold for employing antiarrhythmic agents in the treatment of benign or potentially lethal ventricular arrhythmias. Many have managed patients by switching to the traditional class I quinidine despite its known proarrhythmic tendency. METHODS AND RESULTS To evaluate the relation ...

Treatment of resistant atrial fibrillation. A meta-analysis comparing amiodarone and flecainide.

BACKGROUND Chronic atrial fibrillation (AF) is a common arrhythmia with significant morbidity and mortality. Maintenance of normal sinus rhythm (NSR) can be achieved with antiarrhythmic drug therapy. The antiarrhythmic effects of amiodarone hydrochloride and flecainide acetate in patients with resistant chronic AF have been investigated separately in several small studies. This investigation co...

A proposal for the clinical use of flecainide.

Effective antiarrhythmic therapy requires a carefully considered approach, including an understanding of the arrhythmia, the underlying cardiac disease and the drug's pharmacokinetics. Flecainide is a new antiarrhythmic drug that may soon be released for general use. Flecainide demonstrates unsurpassed efficacy in chronic ventricular arrhythmias in stable patients and may become a first-choice ...

Increased Right Ventricular Repolarization Gradients Promote Arrhythmogenesis in a Murine Model of Brugada Syndrome

INTRODUCTION Brugada syndrome (BrS) is associated with loss of Na(+) channel function and increased risks of a ventricular tachycardia exacerbated by flecainide but reduced by quinidine. Previous studies in nongenetic models have implicated both altered conduction times and repolarization gradients in this arrhythmogenicity. We compared activation latencies and spatial differences in action pot...

Suppression of ventricular ectopic depolarizations by flecainide acetate, a new antiarrhythmic agent.

Flecainide acetate, a new antiarrhythmic agent, was given orally to 11 hospitalized patients with chronic high-frequency ventricular ectopic depolarizations. Drug effectiveness was evaluated with a dose-ranging single-blind protocol, which included placebo control and washout periods. Twice-daily dosing (average daily dose 436 mg) completely suppressed all ventricular ectopic activity in five o...
